I remember reading about a detent pin getting stuck in a shaft not that I know what it does which seemed to match these symptoms, the mechanic said it's a bit of an effort to repair so only worthwhile when doing a clutch change, mines on 156k and the clutch shows no sign of going yet!. I'm in Ireland and these gearbox's are rarer then hen's teeth. Far easy moduation and you actually feel in control of the clutch. But if you get the 7 sears or snap on bolt removal tool, you can get it off. The additional security measures brought an additional weight of 130 kg 287 lb compared to the normal 540i sedan.
I can get them for little money. It was clearly designed as an Auto and the manuals seem like an afterthought Never had any problems with reverse, however agree with sometimes not fully selecting first properly and sometimes third too. The M57 was a diesel, with substantial torque output. I find i have to be about 3000rpm or higher to get a smooth enough gear change from 1st to second!
